The Courier still resides in New Vegas and the Great Khans have taken over most, if not all of Wyoming along side the Followers of the Apocalypse.
If Bethesda is to ever go back to the West Coast, they should just say that "The Legion lost, Caesar's dead, Lanius was talked down and now controls what little bit of land didn't revolt after Caesar died and the Legion broke apart as Caesar is the only thing they are built on as he is the "Son of Mars". The Think Tanks from Big MT have an extremely high chance of competing House's space program (which is a major reason many people side with him) if The Courier has high Karma. I'd imagine the best ending Bethesda could choose to have is House for story reasons, but NCR is the best personally, but they should leave it ominous.
